Staff and patients no longer need to wear a face mask when working at, or visiting the surgery. However if patients are visiting the surgery with respiratory problems, with a fever or choose to wear a face mask and request the clinician wears a face mask we ask that you then wear a mask.
If a member of staff request you to wear a mask because they are unsure about your symptoms please compile with this request. We will continue to be vigilante and if there is any change in the local rates of covid infection we will review this guidance and reserve the right to reintroduce measures to our premises.

Patient Access allows you to order repeat prescriptions, as well as book GP appointments and find local health services for you or your family via your mobile or home computer.
Sleights and Sandsend Medical Practice
is part of the Whitby, Coast and Moors Primary Care Network (PCN). PCNs have been created to help integrate primary care with secondary and community services and provide care at greater scale. In doing this we can develop the services we offer to meet the needs of our local population.

How can I request a sick/fit note?
If you are off work sick for seven days or less, your employer should not ask you for a doctor’s certificate. You will need a medical certificate when you’ve been off more than seven days. The seven days include days that you don’t normally work. So when you work out how long you’ve been off sick, you should include weekends and bank holidays.
A medical certificate note must be signed by a doctor. The Fit Notes mean your GP can give you advice to help you return to work. This is because work can play an important part in helping people to recover from illness on injury. The GP can either say you are not fit for work, or can say you may be fit for work.
If you have seen a doctor at the practice regarding the problem you need a medical certificate for (or we have received a letter from the hospital about your sickness) you may not need to see the doctor again. You can complete a fit note request form at reception, which will be passed to the doctor to review. The doctor may also be able to speak to you on the telephone depending on how well they know you and why you are off work sick.
How can I register for online services
You can now use the internet to book appointments with a GP, request repeat prescriptions for any medications you take regularly and to look at allergies and adverse reactions within your medical record. You will need to think of a password which is unique to you. This will ensure that only you are able to access your record – unless you choose to share your details with a family member or carer.

How do I register as a new patient?
If you are thinking about registering as a patient with us please check our practice area before completing the form below as we can only accept patients who reside at a residential address within the practice boundaries.
You may also apply by attending either surgery where the receptionist will check your residential address and if you are eligible to join the practice you will be given the necessary forms to complete.
