Register as a patient
If you are thinking about registering as a patient with us please check our practice area before registering. Please use the online links to register with the surgery.
You may also apply by either completing the registration form below or by attending either surgery where the receptionist will check your residential address and if you are eligible to join the practice you will be given the necessary forms to complete.
The register online – Churchfield link below enables you to register with the Practice, once registered you can attend the surgery of your choice. Doctors, nurse and HCAs work across both sites. If you are a dispensing patient you will be able to allocate your preferred site for collection of prescriptions.
If you are returning a completed registration form to the surgery please ensure you bring two forms of identification, one of which preferably a form of picture ID.
The NHS requires that every patient is allocated a named accountable GP. All registered patients have been allocated a named GP, and new patients are allocated a named GP upon registration. This is for administrative purposes only and you retain the right to book an appointment with the GP or other clinician of your choice. We encourage patients to ask for the GP dealing with their care when making an appointment, for continuity and we will do our best to facilitate this.
New patient check
Once you have provided us with your completed registration form, health questionnaire and identification, please make an appointment with our Nurse or Health Care Assistant for a ‘new patient check’. As part of this check up, we will take basic measurements such as blood pressure, height and weight.
